Before Your Appointment

Review the above to help yourself prepare for your appointment.

Below you will find a explanation of how to prepare for your appointment with the Toronto Concussion Clinic.  These procedures are meant to help patients and our concussion team to get prepared for your appointment.  If you have any questions feel free to send us an email with your question to team@torontoconcussion.ca.  Also, you are welcome to attend our OHIP-Funded Virtual Orientation Groups Sessions where you’ll be able to ask any question you may have about the Toronto Concussion Clinic or concussion treatment in general!  If you would like to apply for a free OHIP-Funded Concussion Appointment with one of Concussion doctors, please complete our Request a Consultation Form and our staff will reach out to you about next steps.

Have someone help you complete the questionnaire(s) if computer/screen work bothers you or if were instructed not do the same.  Please fill out the online questionnaire(s) we send you at your convenience.  If there is more than one page in the questionnaire, you will be asked to fill out one page at a time.  When you have completed a specific page, you can click on “Next” and that page of the questionnaire will be automatically saved.  You can then decide if you want to do the next page of the questionnaire (pops up automatically) or close the browser and continue at another time.  Please note that if you want to save any of your answers in a questionnaire, you will have to hit “Next”, otherwise, your answers will not be saved.  Also, if you do not complete all questions of a questionnaire before you click “Next” you will not be able to go back and complete unanswered questions the next time you log in to the portal.

The next time you want to log into the portal, you can access it through the same email link that was initially sent to you. The questionnaire(s) will only be uploaded to your chart when ALL pages are complete and you have clicked “Next” on all of them.

The office will then call you for a follow-up appointment afterwards.
